The standard shape is a set of dungeons at increasing difficulty, each with its own loot table, and a gear requirement to enter the harder ones. Because each run is instanced, the loot inside is not contested by other groups and the encounters can be scripted properly. Progression is a ladder of dungeons rather than a mining or building ladder.
